The abortion pill (the two-pill process) is the most common medication abortion procedure used in the US to end a pregnancy. The FDA has approved this procedure for women up to 10 weeks of pregnancy. This procedure is also known as a chemical abortion, medication abortion, at-home abortion, telemed abortion, and more – but they are all referring to the same method of an early abortion. Procedure

The Medication Abortion is a process.

  1. An assessment will be performed at an abortion clinic, and then the patient will be given the first pill at the clinic. This first pill blocks the production of progesterone1, a hormone necessary to support a pregnancy. This works to cut off the lifeline to the baby.
  2. Next, patients get their second dose of a different medication1 to take at home two days later. You can expect cramping and bleeding for 6-19 days, or even up to 30 days in this phase, which works to expel the embryo.
  3. If the medication abortion fails, a surgical abortion will be necessary. You will return to the abortion provider within a week to 14 days to assess whether or not the medication abortion procedure worked and if you will need to have a surgical abortion.  

Side Effects, Risks & FDA Warnings

Side effects include weakness, headache, fever/chills, vomiting, diarrhea and dizziness for up to 3 days2.

Serious side effects include excessive bleeding, pelvic infection and failure to terminate pregnancy1.

If the medical abortion fails to terminate the pregnancy, your doctor may perform an ultrasound to determine if you require a surgical abortion1.

The FDA warns women to only obtain this drug from certified prescribers. Some of the requirements for becoming a certified prescriber are: the ability to ascertain how many weeks along the pregnancy is and confirm that it is not ectopic and also to be able to provide or refer for surgical intervention and emergency services, if necessary2. Do NOT buy the abortion pill from just anyone over the internet. For your safety, you should continue to follow up with your physician throughout the process. 

The FDA2 warns that women with an ectopic pregnancy (implantation in the fallopian tubes) or women who have an IUD in place should not take this drug. Women taking blood thinners or those who have medical conditions such as seizure disorders, bleeding disorders, or certain heart, liver, kidney or lung diseases are not candidates for the abortion pill.

New Abortion Pill Option

Recently, some organizations in the U.S. have begun to dispense another medication abortion option than the one above, even though it is not yet approved by the FDA to be used alone as an abortifacient. This option makes use of the second pill only and can be used up to 12 weeks of pregnancy. 3-4 doses of the pill are taken every three hours, resulting in bleeding and cramping to expel the fetus. This typically lasts several hours, though the bleeding can continue for two weeks. Common side effects are nausea, vomiting, diarrhea, fever and chills. Be sure to discuss possible serious side effects with your doctor, so you know what to look for. In addition to a higher chance of side effects, this method is also less effective than the two-pill method approved by the FDA and is more likely to require further measures to terminate your pregnancy. As with the two-pill method, it is critical for your safety that, before taking any abortion pills, you have an ultrasound to determine how far along you are in your pregnancy and the location of the fetus. Fetal Development – First Trimester

The following developmental benchmarks3 occur during the same time in which the abortion pill is offered to women.

Weeks 1 – 4

  • The embryo’s heart is beating
  • All major organs and internal systems begin to form
  • The digestive, heart and circulatory systems begin to form, as well as the neural tube (which later becomes the brain & spinal cord)
  • Eyes and ears begin to develop
  • Tiny limbs appear, which will grow into arms and legs

Weeks 4 – 8

  • All major organs continue to develop and the baby’s heartbeat can be detected using a Doppler
  • All major systems are functioning and continue to develop
  • Bones are forming, nose and jaw rapidly developing
  • Eyes, ears, nose and mouth becoming more clear and tooth buds begin to appear
  • Arms and legs are clearly seen and fingers and toes, though still webbed, are more distinct
  • Embryo is in constant motion and fluttering will be felt in the next few weeks

Major Developmental Milestone at the End of Week 8

  • At just 1 – 1.5 inches long, all major systems and organs are fully formed
  • The embryo is now referred to as a fetus, which means offspring

Weeks 8 – 12

  • By the end of week 12, the external genital organs are clearly visible and developed
  • From weeks 8 – 12, fingernails and toenails appear, eyelids form, arms and legs are fully developed, and fetal voicebox, also known as the larynx, begins to form in the trachea.
